Falling from a ladder in CS 1.6 with 1 HP.

Labyrinth Zone.
Trapped underwater.
Running out of oxygen.
See a bubble.
Run to bubble.
Die when half of face is in the buuble, although it doesn't quite pop it.

in kingdom hearts running away from a monster and getting killed from 10 feet away.

In Oblivion, I was a vampire and I was running around in the middle of nowhere, getting damaged by the sun. It doesn't end like you think it does, though. I jumped over a rock and died upon landing. I effing jumped and died. I DIED BECAUSE I JUMPED FIVE EFFING FEET.

I sat there for a straight minute, shaking my head and staring at the screen, before I threw my controller against the wall and stormed out of the room screaming and cursing like a sailor.
